Senate Bill 66 applications mailed

FRANKFORT — Senate Bill 66 applications have gone out and are due to be postmarked by January 15, 2008. Service providers should be aware that their judges should have the form in their office now.

Application forms can also be downloaded from the Kentucky Board of EMS' web site.

Remember that, as of Friday, Dec. 14, KBEMS will no longer be at their Frankfort address and that all mail and these applications should be sent to KBEMS, 300 North Main Street, Versailles, KY 40383.
