  • CON #054-02-3232(5) COMMUNITY CARE AMBULANCE SERVICE, Madisonville, Hopkins County. Establish a class 1 specialized (BLS) ambulance service utilizing one vehicle to serve Hopkins County. $35,000.00 1/30/02
