Certificate of Need actions

 

The following items were published in the September edition of the Kentucky Cabinet for Health Services Office of Certificate of Need Newsletter.

Formal review proposals scheduled for decision Dec. 20:

  • CON #054-02-3232-(4) Community Care Ambulance Service, Madisonville (Hopkins County). Establish a Class I BLS emergency ground ambulance service utilizing two vehicles to serve Hopkins County. ($100,000.00)
  • CON #052-06-3671-(1) Henry County EMS, New Castle (Henry County). Establish a Class I BLS ambulance service utilizing two vehicles currently licensed to Henry County EMS #3 to serve all of Henry County. ($10,000.00)
  • CON #041-07-3687-(1) Dry Ridge Fire Department, Dry Ridge (Grant County). Establish a Class I ALS/BLS ground ambulance service to serve the City of Dry Ridge and Grant County utilizing two vehicles. ($132,000.00)
  • CON #010-10-2454-(5) Medical Transportation Service, Ashland (Boyd County). Expand existing Class II ambulance service are to include all of Greenup County and add two vehicles. ($121,700.00)
  • CON #022-10-3677-(1) City of Olive Hill Fire-EMS, Olive Hill (Carter County). Establish a Class I BLS ground ambulance service to serve the western half of Carter County to include the City of Olive Hill utilizing one vehicle. ($37,260.00)
  • CON #022-10-3678-(1) City of Grayson Fire Department/Rescue, Grayson (Carter County). Establish a Class I BLS ground ambulance service to serve the eastern half of Carter County utilizing one vehicle. ($49,999.00)
  • CON #007-13-103-(4) Bell County Emergency Medical Services, Pineville (Bell County). Establish a Class I ALS specialized ambulance service to transfer patients from Middlesboro ARH to other hospitals utilizing one vehicle. ($0.00)

Letters of intent:

  • Clover Fork Nonemergency Transportational, Harlan (Harlan County). Establish a non-emergency health transportation service to serve Harlan County. 08/14/2000.
  • Yellow Ambulance Service, Lexington (Fayette County). Establish a Class I ambulance service to serve Fayette County. 09/05/2000.
